Camp313 - Registration form

Bayt al Qaim Islamic Centre in collaboration with Idara-e-Jaaferiya (London) have organised Camp313 under the guidance of Maulana Syed Mohammad Raza Jan Kazmi and Maulana Syed Hassan Abbas Rizvi, during February half-term 16th Feb to 18th Feb 2023. Camp313 as part of "Level 2 - Friends of Imam-e-Zamana (ATFS) programme".

Fee of £10 per child is payable.

    Declarations

    * I will maintain discipline and observe all rules set by the Camp organisers (separate document with all the rules will be provided).
    * All participants will stay within the premises of the Idara for the duration of the Camp.
    * I will let the organisers know if my child cannot be photographed.
    * I will be responsible for keeping all my information up-to-date.
    * It is my responsibility to inform the organisers of any medical allergies related to a participant.
    * I take full responsibilities for my children and will facilitate them on camp to the best of my ability.
    * I am responsible for my own travel to and from the camp.
    * children must be accompanied by at least one parent.
    * I give consent to be contacted by mobile, text and email.
